When to Contact a Case Manager
Common situations where case management guidance may be helpful.

A case manager may be helpful when a person or family needs support understanding service options, planning care transitions, or connecting with community resources.
Referral partners may also contact a case manager when a client would benefit from additional coordination after discharge or during a period of changing support needs.
For emergencies or urgent medical concerns, call 911 or the appropriate emergency service.
